If you received a phone call that offers you Prescription Drug Coverage Possible FRAUD

DO NOT GIVE ANY PERSONAL INFORMATION.

Verify it is a Medicare Approved Prescription Drug Plan in “Medicare & You” handbook.

If you get a mail offer selling Prescription Drug Plan Coverage Possible FRAUD

DO NOT GIVE ANY PERSONAL INFORMATION.

If someone comes to your door to sell you a Prescription Drug Plan Possible FRAUD DO NOT GIVE ANY PERSONAL INFORMATION.

If someone calls and tells you they are from Social Security to verify information from the Extra Help application that was sent to their office It COULD BE REAL, especially if you filled out the Extra Help application from Social Security. They already know your basic information, such as social security number, or the address where you receive your check. Social Security WILL NOT ask for these. They have them. If for any reason you are uncomfortable or unsure if they are with Social Security offices, you can tell them you will call your local offices to verify the information and hang up. Call 1-800-772-1213 and tell them you got a call to verify information on the Extra Help form. They will make sure the form is correct and you know to whom you are talking.
If ANYONE calls and wants YOUR Medicare Number, or your Social Security Number, your Check Routing Number, or ANY Personal Information FRAUD NEVER give out your Medicare Number, your Social Security Number, your Check Routing Number, or ANY other Personal Information.

Incidents and practices which, although not usually considered fraudulent, are inconsistent with accepted sound medical, business or fiscal practices
